Permanent Supportive Housing MMON’s Permanent Supportive Housing (PSH) programs are designed to promote residential stability and positive health outcomes by offering indefinite stay housing options to individuals and families who are experiencing homelessness. Services include rent subsidies, case management, transportation assistance, and advocacy. To determine if you are eligible for Permanent Supportive Housing you must be assessed at one of Houston’s designated Coordinated Access sites. Rapid Re-Housing (RRH) MMON's Rapid Re-Housing (RRH) programs are designed to promote residential stability and positive health outcomes by offering short-term rental assistance and services to individuals and families who are experiencing homelessness. Services include rental assistance, case management, and services. To determine if you are eligible for Rapid Re-Housing you must be assessed at one of Houston’s designated Coordinated Access sites. HOMELESS PREVENTION PROGRAM(HPP) MMON’s Homelessness Prevention Programs provide services to those who are at risk of entering a shelter due to rental arrears. Services may include case management, tenant/landlord mediation, advocacy/benefit entitlements, short-term financial assistance, referrals and varied workshops that can help individuals who are at risk of homelessness. HMIS AND DATA SHARING PRACTICES MMON collects personal information about individuals in a computer system called a Homeless Management Information System (HMIS) for reasons that are discussed in our HMIS PRIVACY POLICY. We may be required to collect some personal information by organizations that fund the operation of our programs. Other personal information that we collect is important to run our programs, to improve services for individuals, and to better understand the needs of individuals. In order to provide or coordinate individual referrals, case management, housing or other services, some client records may be shared with other organizations that are required to have privacy policies in place in order to protect your personal information. Access to HMIS records, case records and other confidential information will be limited to staff members. Confidential information will not be released without a client’s written approval except in the event that a medical, mental health, legal, or other emergency arises in which notification is necessary.
